Absence Request Form

Complete all fields. Your request will be sent to your team's coaching staff for review.

Submission Deadline: This form must be submitted no less than 2 weeks before the absence date. In the case of an emergency, you must still submit this form and contact your coach, All-Star Director, or the front desk directly.

Tardiness & Partial Absence Policy

Any tardiness exceeding 50 minutes is recorded as a Partial Absence (warning). Two (2) Partial Absences equal one (1) Unexcused Absence, which carries the standard $75.00 fee. Arriving on time is your responsibility.

Competition Blackout Period Warning

If your absence falls within 2 weeks before a competition, it is subject to the Blackout Period policy — zero absences are permitted, excused or unexcused. Any absence during a blackout results in removal from the event, a $75 fee, and any replacement fees billed to your account. Check the competition schedule before submitting.

Financial Consequences Apply

Unexcused absences carry a $75 fee per practice missed. For absences close to a competition, refer to the Blackout Period policy above. Submitting this form does not guarantee an excused absence — it must be reviewed and approved by your coach and the All-Star Director.
